What To Expect
After contacting our office, Tracy will mail you our new patient package. It will include forms to fill out and bring with you for your initial appointment. You may also opt to download these forms from our website. We ask that you arrive 15 minutes prior to your appointed time.
Your initial appointment will take approximately one hour. If you are experiencing an immediate problem or pain, we will try to address that issue first. We will perform a full oral exam. If necessary, digital x-rays may be taken, as well as intraoral photos and study models of your teeth. A comprehensive set of issues and solutions will be developed and an individualized custom treatment plan created according to your needs and desires.
What To Bring
In addition to your completed forms, please bring any x-rays that you may have from your referring dentist to help us better evaluate your condition. Please bring a list of current medications and any health history. This is helpful should Dr. Schwartzberg need to prescribe any medication over the course of your treatment.
Insurance
If you have dental insurance, please bring that information and your insurance card with you. We are more than happy to submit all insurance forms and claims to help you recover the most reimbursement from your insurance benefit.
